Keep a Symptom Journal
A simple journal can help you remember patterns that may otherwise be difficult to explain months later.
Record significant symptoms and their frequency.
Note what activities trigger or worsen them.
Document how long recovery takes.
Record particularly difficult days.
Keep the journal factual and consistent rather than trying to write what you think Social Security wants to
hear. A useful record can show patterns over time that may not be obvious from individual medical
appointments.
